In partnership with your healthcare provider, the Uw Zorg Online app grants you convenient access to your medication overview as noted by your GP. You can reorder previously prescribed medications, schedule appointments, and initiate eConsults!
You can view a list of participating practices within the app.
We welcome your feedback and suggestions, which can be shared through the feedback button in the app or by emailing us at apps@pharmeon.nl.
Before starting, your practice must enable this service for you:
1. Download the app.
2. Locate your practice.
3. If you already have an account for your GP’s patient portal, log in using those credentials (proceed to step 4).
If you do not have an account yet, click the "Sign Up" button and complete the necessary information.
Once your application is reviewed by the practice — a process that may take some time — your account will be set up, and you will receive your login information via email.
4. Upon logging into the app, you will receive a one-time verification code via email or SMS.
5. Finally, create a 5-digit PIN within the app to secure access.
6. You can now use the service!
Functionality includes:
• Viewing your current medication profile as recorded by your GP.
• Directly requesting repeat prescriptions from your medication list, along with reminders for future needs.
• Asking medical questions through an eConsult and receiving a response once your inquiry is addressed. Note that eConsult is not for urgent or life-threatening issues; if you're unsure about the severity of your symptoms, always contact your GP directly by phone.
• Checking available times in your doctor’s calendar and scheduling appointments that accommodate your schedule. You will need to specify the reason for your visit.
• Accessing your doctor’s address, contact information, and opening hours in the app, as well as a link to their website.
Privacy
- The app allows you to access your medication data via a secure connection and communicate with your doctor.
- Before activation, your identity will be verified by the practice, and you will receive a verification code to start the app.
- Additionally, you can protect the app with a personal 5-digit PIN.
- Your data will not be shared with third parties; further details can be found on your healthcare provider's website.
